Non-NHS Services

The procedures, consultations and services carried out at this surgery are paid for by the NHS. We currently only have the capacity to offer the NHS standard services.

Our priority is NHS work and ensuring we have the capacity to see patients.

We do NOT offer a paid for private service such as the completion of .... 

Certificates, Forms and Letters:

• Private Insurance forms
• Medical Declation forms 
• HGV license forms
• Bus pass application forms 
• Holiday cancellation forms & medical reports
• Fitness to travel certificates
• To whom it may concern letter
• Copies of scans and results
• Housing Letters of support 

GPs do not provide housing support letters – Sheffield City Council does not require them, and Housing Associations are not entitled to them.
If SCC needs medical evidence, they will contact the GP directly with patient consent.
Patients can seek support from hospital specialists, social workers, or other professionals.

GPs are no longer required to provide professional references for rehousing applications. The absence of a GP reference will not negatively impact applications.

If a professional requires a supporting document, you can request that they email the practice, with information around what is needed to support your request. Where appropriate we may be able to help with this.

Alternatively, you can request a subject access request (SARS) of your medical records, your first application for this is free and will be provided to you within 28 days. 

Private prescriptions

We do NOT offer a private vaccination service, such as travel vaccinations and advice. These must be sought by an external provider which can be found online.
