DIABETIC REVIEWS
All patients with Diabetes will be reviewed at least once a year, either at the practice or the hospital.  Please make an appointment if you are having any problems.

ASTHMA REVIEWS
A Practice Nurse will invite everyone with asthma for review at least once a year.  Please make an appointment if you are having any problems.

HEART DISEASE AND STROKE REVIEWS
A Practice Nurse will invite you for a review at least once a year. Please make an appointment if you are having any problems for example more frequent chest pain.

SPIROMETRY CHECKS
is available as requested by a GP or Nurse.

WELL MAN CHECKS
for lifestyle advice, blood pressure, weight, diabetes, cholesterol screening and heart disease prevention.

WELL WOMAN CHECKS
as above plus smears and advice on women’s health issues.

75 PLUS CHECKS
If you are aged 75 years or over, a nurse will invite you to an annual health check and ask you to complete a health assessment questionnaire.  If you are housebound a district nurse will offer you this check at home.

FLU VACCINATION
An influenza vaccination is particularly recommended for patients with heart, lung or kidney disease, diabetes, for residents of nursing and rest homes and everyone over the age of 65. Notices will be displayed in the waiting area from the end of September informing you of the date and times of the clinics. Please ring the surgery to arrange your appointment.

Home visits can be arranged for housebound patients.

TRAVEL VACCINATIONS
Please make an appointment with a practice nurse at least 6 weeks in advance of your holiday, to make sure that you are well protected.  If you leave insufficient time we cannot guarantee that the vaccinations will be effective, or that we will be able to fit you in. 
Most vaccines are available on the NHS, but for those that are not, a charge will be made.  A list of these charges is displayed in the reception area.

SMOKING CESSATION

CHIROPODY
Chiropody clinics are held at Firth Park Clinic. A GP or nurse will refer you if you need an appointment.

YOUNG PATIENTS
Young patients can see a doctor or nurse on their own if they want confidential advice.

EMERGENCY CONTRACEPTION
The practice offers emergency contraception, which can be taken up to 72 hours after having unprotected sexual intercourse. 
If there are no routine appointments, please ensure Reception staff know that you require this service and they will arrange for you to be seen promptly.

COUNSELLING
We have three staff who offer different counselling services.  If your doctor feels you may benefit from seeing one of them, they will discuss with you which would be the most appropriate and make a referral.

OCCUPATIONAL HEALTH
At least one in ten of the labour force reports work-related ill-health, and one in five state they are moderately to severely stressed by work.  If you need to discuss any of these issues, our Occupational Health worker would be happy to speak to you.  Ask Reception staff to book you an appointment.

CARERS
If you care for a family member or friend with a disability, illness or frailty, or a child with special needs please inform the doctor or nurse when you see them.  We like to highlight this in your notes so we can offer you support and flexibility when you are accessing our services.  We also suggest you register with the Carers’ Centre – ask Reception staff for details.
